The National Transitions of Care Coalition is pleased to announce the release of the Transition of Care Compendium. The Transition of Care Compendium can now be accessed on the NTOCC website. The Compendium is a collection of white papers, journal articles, and websites consisting of resources that transitions of care-related professionals or interested consumers might find useful in a practice or medical situation.
