Seeking staff from health plans, AAAs, other aging care organizations, providers, state agencies, Centers for Independent Living or any other organization who have supported disabled people of color in transitioning from institutions back into the community.

Brandeis University is doing a research study about nursing home transition for disabled people of color. Participants will be asked to: 1) complete an online intake form and 2) participate in one 90-minute virtual focus group. You will receive a $50 gift card for participating. 

Information shared will be compiled into research that can be used to inform future policy.
