Conference Proceedings from LTQA’s 3rd Annual Meeting, “Building Bridges across the Continuum to Achieve Person-Centered Care,” are now available.

Attendees from around the U.S. came together to share best practices and broaden their knowledge of innovative approaches in delivering and financing long-term services and supports, and to discuss 

• Blending medical and long-term services and supports;
• Managing and optimizing the costs of people with long-term care needs;
• Using data to promote continuity of care and increase accountability; and
• Redesigning payments at the state and local level.
