Five national health care foundations – The Commonwealth Fund, The John A. Hartford Foundation, Peterson Center on Healthcare, the Robert Wood Johnson Foundation, and The SCAN Foundation – have partnered with the Institute for Healthcare Improvement to create the Playbook, a website containing resources about care approaches and policies to better support individuals with complex health and social needs.
